Provider Growth and Relationship Lead
Community Outreach & Referral Partner Development
Actively manage and grow patient referrals in your region through systematic outreach to existing provider partners.
Identify and develop new potential referral sources to grow Amae's presence.
Collaborate with hospital systems, social workers, and providers to coordinate rapid responses to urgent patient situations.
Work closely with clinical staff to ensure referred patients are connected with appropriate care.
Serve as a visible, trusted ambassador for Amae Health in the community.
Planning, Collaboration, & Documentation
Track outreach activity, referral outcomes, and pipeline in CRM to meet or exceed enrollment targets.
Develop quarterly market plans in collaboration with clinical and leadership teams.
Participate in weekly team meetings; complete documentation accurately and on time.
Qualifications
- 3+ years of client or customer-facing experience with defined engagement targets
- Proven track record of building a referral base and relationships in business development, sales, community outreach, or referral management
- Knowledge of and passion for mental health and wellness
- Excellent oral and written communication skills, including strong public speaking ability
- Reliable transportation and ability to travel throughout the Bay Area (supporting our Los Altos and San Mateo clinics)
- General computer proficiency and ability to learn tools like Google Suite, Slack, and CRM systems
