Essential Functions/Responsibilities
- Provide programmatic support where appropriate for site, staff and participants.
- Submit and monitor required documentation to Behavior Department for individuals requiring more intensive behavior services.
- Collaborate with Senior Day Supervisor to provide suggestions and support for ongoing behavior plans.
- Review training provided by the Behavior Department associated with behavior plans for staff.
- Initiate communication and update site management on concerns and report on follow-up actions.
- Ensure the Behavioral Health Planning and semi-annual reviews are conducted on-site.
- Ensure that Informed Consents and Site Based Restrictions are updated annually.
- Monitor and ensure the implementation of fading plans from MMP’s/BSP’s.
- Daily completion and review of eVero DA Logs, Behavior Tracking, Helmet Tracking, Sweeps Tracking, and Restrictive Intervention Applications (RIA), using the data to facilitate program planning.
- Consult with site BIS to present HRC packets for individuals without active BIS support.
- Prepare and submit HRC packets to PD and attend HRC meetings.
- Collaborate with team members including SDS, PD, RN to ensure all scripts are updated and available on site.
- Support crisis intervention as needed to assist direct care staff.
- Ensure supplies and supports are available as per individualized behavior plans or protocols.
- Attend and participate in general staff meetings, in-service training, monthly support team meetings, and other pertinent agency and community program meetings.
- Maintain attendance at Life Plan meetings as needed.
- Keep records of all site in-services and trainings.
- Assist in developing Day Habilitation staff action plans for assigned individuals.
- Comply with all agency policies and procedures.
- Perform other duties as assigned.
Minimum Qualifications
- A./B.S. or associate's degree in psychology or behavior management related field preferred, with two years of experience or combination of Education and Human Service Experience.
- Minimum of 2 years related experience.
- Ability to meet physical requirements of the position, including standing, walking, sitting, bending, stooping, squatting, kneeling, and climbing.
- Lifting requirements of 35 lbs.
- Ability to lift and transfer individuals manually and mechanically as required by prescribed program plan.
- Physical agility and ability to react to emergency situations, including maintaining required certifications (SCIP, CPR).
- CPR certification required.
- Behavior Technician Training required.
- Medication certification required.
- Valid NYS Driver’s license meeting agency policy.
