Responsibilities
- Attends home visits, with the case planner and independently, throughout the life of the case.
- Reports any concerns about child safety and wellbeing.
- Accompanies parents to service appointments and to programs.
- Supports service plans in the areas of effective parenting practices and concrete services such as benefits and entitlements.
- Acquires school and medical records in support of service plans.
- Follows up and reports on service plan tasks and activities by clients.
- Documents all work provided on program forms, in PROMIS and CNNX databases.
- Attends weekly supervision, staff meetings and case conferences.
- Provides direct services to clients, including intake, engagement, assessment, referral, service plan development, advocacy, counseling, exit summaries and follow-up.
- Makes sure client progress is documented toward service plan goals.
- Conducts home visits and field work, as required.
- Documents all interactions with, or on behalf of, clients.
- Provides general clerical support, including filing, faxing, word processing, typing, photocopying and sorting mail.
- Answers telephones, takes messages and directs calls to the appropriate parties.
- Reports to, and meets with, supervisor monthly.
- Completes program reports monthly or more frequently as required.
- Affords assistance with special projects as required.
Qualifications
- Associate’s degree in a related field of study or some college course work completed.
- A minimum of two years of relevant work experience.
- Proficiency in English required; proficiency in a second language preferred.
- Strong oral and written communication, time management and organizational skills are necessary.
- Proficiency in Microsoft Office suite and other standard business technology is required.
