Program Coordinator - RN - Stroke

About the Role

The Facilitator RN collaborates with the interdisciplinary team to oversee and coordinate stroke care, ensuring high-quality patient management during and after the Code White process. This role involves facilitating all aspects of stroke treatment, supporting clinical pathways, and maintaining patient safety and care standards.

Responsibilities

• Manage and coordinate stroke care activities across the care continuum
• Support clinical pathways and evidence-based practices
• Assist during emergency response processes, including Code White events
• Ensure effective communication among healthcare team members
• Monitor patient progress and contribute to quality improvement initiatives
• Provide education and support to staff and patients as needed

Requirements

• Registered Nurse (RN) licensure with relevant experience in acute or stroke care
• Strong clinical knowledge of stroke management and emergency protocols
• Excellent communication, teamwork, and leadership skills
• Ability to coordinate complex care processes under pressure
• Commitment to patient safety and quality improvement
