About the role
Lifeworks is a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. As a Program Coordinator, you will coordinate, develop, and implement rehabilitation services for individuals served by Lifeworks Employment Services.
Responsibilities
- Promotes optimal health for individuals through routine, preventive, and emergency health care and clinical services.
- Ensures that work environments are safe, secure, and where individuals are free from harm.
- Supervises and performs case management responsibilities, including preparation, implementation, and evaluation of Individual Support Plans for employees funded by the Department of Developmental Services, Massachusetts Rehabilitation Commission, School Departments, etc.
- Works closely with the DDS Case Management Team Service Coordinators.
- Serves as the program Safety Officer as assigned by Director and be responsible for implementing safety plans and programs.
- Encourages regular communication and cooperation between staff of all LES programs to ensure coordination of rehabilitation and production efforts.
- Maintains effective communication with residential providers and families regarding activities and supported employment placements.
- Covers for various sites, placements, activities, and outings in the community.
- Provides training, support, and follow-up to individual and group placements.
- Maintains accurate records for individuals served for the purpose of program evaluation, billing, etc.
- Coordinates accurate payroll records for all individuals and employees, job coaches, and relief staff.
- Aids the leadership team in long-term and short-term planning development, organization, and evaluation of services and in the daily operation of the program.
- Implements Quality Enhancement standards within the program.
- Coordinates the scheduling of all relief staff for the Program.
Requirements
- Experience working with Individuals with ID/DD
- Bachelor's degree from four-year college or university and one to two years related experience
- Pass Fingerprint background check, CORI, DPPC, and Driving background checks
- Valid Driver's License required
- Excellent verbal and written communication skills
