About the role

The Re-entry Program Coordinator works under the direction of the Program Director and Program Manager to ensure contract compliance and maintain the quality of services provided by the St. John’s Re-entry Program. This position supports the mission of St. John’s Community Health by overseeing the daily operations of the Re-entry Team and ensuring high-quality service delivery to patients, staff, and funding partners.

Responsibilities

  • Supports the mission of St. John’s Community Health by overseeing the daily operations of the Re-entry Team and ensuring high-quality service delivery to patients, staff, and funding partners.
  • Strategic site coordination, deploying teams based on community needs, and facilitating timely, effective communication with the RISE Director.
  • Developing and maintaining operational workflows, managing documentation and data reporting, supporting financial tracking and reporting, and serving as the primary point of contact for team members.

Qualifications

  • Associate’s or Bachelor’s degree in public health, social work, human services, or a related field preferred; equivalent work experience will be considered.
  • Minimum 2 years of experience in program coordination, administrative support, or related roles in a nonprofit or community health setting.
  • Strong organizational and time management skills with the ability to manage multiple priorities and meet deadlines.
  • Excellent written and verbal communication skills; ability to communicate effectively with diverse staff, community partners, and patients.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook, Teams); experience with data tracking and reporting tools preferred.
  • Knowledge of or willingness to learn about social determinants of health, trauma-informed care, and services for vulnerable populations.
  • High attention to detail, especially in documentation, reporting, and file organization.
  • Ability to work independently and collaboratively in a team-oriented, fast-paced environment.
  • Demonstrated cultural humility and sensitivity when working with diverse populations.
  • Flexibility, adaptability, and a proactive approach to problem-solving.
  • Understanding of confidentiality and compliance requirements in community health and human services programs.
  • Experience supporting audits or compliance reviews is a plus.
