Nexus Family Healing is looking to add several PRN Registered Nurses to our residential program in Portland, Oregon. This small Psychiatric Residential Treatment Facility (PRTF) serves up to six youth at a time, ages 10-17 with mental and developmental disabilities who have become involved in the justice system. The program, located at Nexus’ Walden Crossing, is a secure residential location providing high staff-to-youth ratios, 24/7 nursing, recreational and therapeutic professionals, psychiatric services, and an array of therapies as indicated.

About the Role

Our Nurse (RN) promotes the health and well-being of Nexus Family Healing residents by providing client-centered health care services in accordance with Nexus policies, procedures, and protocols.

Responsibilities

  • Resident Medical Care
    • Collaborates with interdisciplinary teams regarding medical aspects of client admissions.
    • Communicates medical concerns and coordinates necessary follow-up appointments and/or referrals with interdisciplinary teams both internally and externally.
    • Collects and reviews clients’ medical data and ensures that medical records are current and accurate.
    • Assesses nursing implications of all data pertaining to the resident and determines the need to communicate with the physician.
    • Monitors acute and chronic medical conditions of clients; provides treatment or consultation as needed.
    • In emergency situations, gathers relevant information, assesses client status, provides appropriate care and/or treatment, directs others as necessary, and collaborates on referral to a higher level of care if appropriate.
  • Treatment Team Collaboration
    • Serves as a resource to staff and other interdisciplinary teams regarding client medical concerns.
    • Collaborates with the treatment staff to develop individual client treatment plans.
    • Implements, monitors, and coordinates medication administration to clients.
    • Verifies, counts, and administers medications to residents.
    • Educates clients on medication side effects, adverse effects, and drug interactions.
    • Monitors clients daily for side effects, adverse effects, and/or drug interactions.
    • Ensures client prescriptions are filled and the appropriate amount is maintained through each client’s stay within the residential milieu.
    • Monitors medication logbooks for completion and accuracy.
  • Staff Meetings & Required Training
    • Completes all required trainings that are department/role specific and facility-wide requirements.
    • Maintains and stays up to date with trainings and certifications (e.g., online trainings, internal and external in-person trainings, CPR certification, nursing license, first aid and safety, medication administration, and infection control).
    • Participates in internal training opportunities with community partners.
  • Client Confidentiality
    • Ensures client information remains confidential and compliant with Nexus policies involving client privacy, informed consent, and confidentiality standards for clients and their families.

Requirements

  • Graduate of an accredited school of nursing.
  • Valid Registered Nurse License.
  • QMHP qualified in Oregon.
  • 1+ year related nursing experience.
  • First Aid/CPR/Infection Control certification.
  • Valid driver’s license.
  • Ability to build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Openness to meeting the cultural needs of individuals or communities, considering factors such as language, traditions, and values.
  • Successfully participate and complete a three-part Nonviolent Crisis Intervention exam and training.

Preferred Qualifications

  • 1+ year related nursing experience in a residential setting.

Schedule

  • PRN (as needed) on-site opportunity at Walden Crossing.
  • Address: 722 NE 162nd Avenue, Portland, OR 97320.
  • Non-hospital setting; small residential program.
  • Working with youth clients ages 10-17.
  • Shift Differential:
    • $1/hour differential for overnights during the week (Sunday night through Friday morning: 12 AM – 7:59 AM).
    • $2/hour for day and evening shifts on weekends (Saturday morning through Sunday evening: 8 AM – 11:59 PM).
    • $3/hour for weekend overnights (Friday night and Saturday night: 12 AM – 7:59 AM).

Pay

Starting pay range: $47-$50 an hour.
