Private Duty Nurse LVN - Nights (Part Time / Full Time)

About the Role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, working under the supervision of a Registered Nurse. The role involves delivering and documenting patient care in accordance with care plans and physician orders, assessing patient conditions, and reporting changes promptly.

Responsibilities

• Deliver and document patient care in accordance with care plans and physician orders
• Assess patient conditions and report changes promptly
• Maintain accurate documentation
• Implement care plans
• Educate patients and families
• Adhere to infection control standards
• Monitor patient status, document activities, update care plans, and ensure compliance with healthcare standards and regulations

Qualifications

• Active, unrestricted LPN/LVN license in the practice state
• Current CPR certification
• TB test within the past 12 months
• Reliable transportation
• Preferred: Six months of clinical nursing experience; home health experience is a plus

Physical & Environmental Requirements

• Ability to lift up to 50 pounds and reposition patients
• Prolonged walking, standing, bending, and climbing stairs
• Ability to travel and work in varied environments, with exposure to bodily fluids and infectious diseases
