Private Duty Nurse LVN - Night Shifts (Part Time / Full Time)

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in patients' homes, under the supervision of a Registered Nurse, ensuring quality care per physician plans and adhering to confidentiality and professional standards.

Responsibilities

• Delivering and coordinating patient care in compliance with orders
• Monitoring and assessing patient conditions, reporting changes promptly
• Documenting assessments, nursing actions, and care activities accurately and timely
• Participating in and updating nursing care plans
• Implementing appropriate interventions based on assessments
• Recognizing and responding to patient needs to prevent risks
• Providing patient and family education on disease management and self-care
• Adhering to infection control protocols and safety standards
• Maintaining professional development through ongoing education

Qualifications and Skills

• Current, active license (LPN/LVN) in the practice state
• Current CPR certification
• Reliable transportation and ability to travel
• Proficiency in clinical assessments, documentation, and compliance
• Attention to detail, effective time management, strong communication, and problem-solving skills
• Physical ability to lift up to 50 pounds, reposition patients, and perform manual tasks
• Preferred experience: Six months of recent clinical or home health experience.
• Physical and environmental requirements: ability to work in diverse settings, exposure to blood and bodily fluids, and respond effectively to emergencies.
• Employment compliance with health-related requirements is mandatory for Medicare/Medicaid providers.
