Private Duty Nurse LVN - Days/Nights

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse.

Responsibilities

• Delivering and documenting patient care according to physician orders and care plans
• Monitoring and assessing patient conditions, reporting changes promptly
• Maintaining accurate documentation of nursing activities and assessments
• Educating patients and families on health management and disease prevention
• Ensuring infection control standards and confidentiality are upheld
• Participating in ongoing education and skill development

Requirements

• Active, unencumbered LPN/LVN license in the practice state
• Current CPR certification and TB test within last 12 months
• Proficiency in clinical assessments, documentation, and compliance with regulations
• Reliable transportation and physical ability to lift up to 50 pounds and move patients/equipment
• Strong attention to detail, organization, communication, and problem-solving skills
• Minimum six months of recent clinical nursing experience preferred
• Experience in home health is a plus
• Ability to work in diverse environments and respond to emergencies effectively
