About the Role
Provide skilled nursing care in home settings, delivering and documenting patient care according to care plans and physician orders.
Responsibilities
- Assess patient conditions and report changes to healthcare team
- Update nursing care plans as needed
- Educate patients and families on care procedures
- Ensure infection control and maintain confidentiality
- Comply with all regulations and participate in ongoing education
- Use electronic documentation tools for accurate record-keeping
- Coordinate care to achieve positive clinical outcomes
- Adapt to various home environments while maintaining professional standards
Requirements
- Active, unrestricted LPN/LVN license in the state of practice
- Current CPR certification
- TB test within the last 12 months
- Reliable transportation
- Physical ability to lift and reposition patients
- Ability to travel and respond to emergencies
- Must meet state-specific requirements and adhere to infection control standards
Qualifications
- Six months of clinical experience preferred
- Home health experience advantageous
