About the role
Join a compassionate healthcare team as a Licensed Practical / Vocational Nurse (LPN/LVN).
Responsibilities
- Providing and documenting skilled nursing care under supervision, following care plans and physician orders.
- Assessing patient conditions, reporting changes, and implementing appropriate interventions.
- Updating and participating in care planning, ensuring compliance with policies and regulations.
- Educating patients and families on disease management, self-care, and prevention.
- Maintaining infection control standards and ensuring patient safety.
- Delivering quality patient care, monitoring patient status, documenting activities, and ensuring safety and compliance in home healthcare settings.
Qualifications
- Active, unrestricted LPN/LVN license in the practicing state; compact licenses transferable within 90 days.
- Current CPR certification and TB skin test within the last 12 months.
- Preferred: Six months clinical experience and home health background.
Skills
- Strong attention to detail, time management, problem-solving, and communication skills.
- Physical ability to lift 50 pounds, reposition patients, and perform clinical tasks.
Requirements
- Reliable transportation and the ability to travel.
- Ability to work in diverse environments, with exposure to bodily fluids and infectious diseases.
