Role Overview

The Licensed Practical / Vocational Nurse (LPN/LVN) provides skilled nursing care under supervision, following care plans and physician orders. Responsibilities include assessing patient conditions, documenting nursing activities, implementing care plans, and educating patients and families on health management. The role requires maintaining confidentiality, adhering to policies, and ensuring infection control standards.

Key Duties

• Deliver and coordinate quality patient care in compliance with physician instructions.
• Monitor and report changes in patient conditions promptly.
• Document assessments, actions, and responses accurately and timely.
• Participate in updating and implementing care plans.
• Recognize and respond to patient needs to prevent risks.
• Educate patients and families on disease processes and self-care techniques.
• Adhere to infection control, safety regulations, and professional standards.

Qualifications & Skills

• Active, unencumbered LPN/LVN license
• Current CPR certification
• TB test
• At least six months of clinical experience (preferred)
• Home health exposure (preferred)
• Reliable transportation (preferred)
• Attention to detail, time management, problem-solving, communication, and organization skills

Physical & Environmental Requirements

• Ability to lift 50 pounds
• Ability to travel
• Function in various environments, including exposure to bodily fluids and infectious diseases
• Meet physical and mental demands of patient repositioning, emergency response, and continuous activity
