About the role

Provide high-quality homecare for pediatric patients with complex needs, including Gtube and Trach care. Ensure clinical outcomes through compassionate, personalized care and collaborate with a multidisciplinary team. Utilize electronic documentation tools in patients' homes and support families with education and guidance.

Responsibilities

• Deliver compassionate homecare for pediatric patients with complex medical needs
• Maintain compliance with health regulations and standards
• Document care electronically using patients' home-based systems
• Support patient and family needs, ensuring safety and comfort during care delivery

Requirements

• Active, unencumbered LPN/LVN license in the practicing state (compact licenses must be transferred within 90 days if applicable)
• Current CPR certification with hands-on component (training assistance available)
• TB skin test within the last 12 months
• Reliable transportation
• At least six months of hands-on nursing experience preferred
