About the role

Responsibilities

• Deliver direct patient care following physician orders and care plans.
• Document patient progress electronically using provided tablets.
• Collaborate with families and healthcare teams to optimize care.
• Maintain compliance with licensing, certification, and health requirements.

Qualifications & Skills

• Active, unencumbered LPN/LVN license in the practice state; compact licenses transferred within 90 days if applicable.
• Current CPR certification with hands-on component.
• TB test within the last 12 months.
• Reliable transportation required.
• Preferred but not mandatory: six months of hands-on nursing experience.
