Primary Care Provider - Advanced Practice Clinician (NP or PA)

About the role

We are seeking an experienced Advanced Practice Clinician (NP or PA) to independently manage a primary care patient panel in a collaborative, value-based care environment.

Responsibilities

• Provide longitudinal, relationship-based care through comprehensive assessments, diagnoses, and treatment
• Manage acute and chronic conditions and develop patient care plans
• Make clinical decisions regarding in-office management or referrals
• Handle inbox duties such as test results, messaging, medication management, and referrals
• Coordinate care with interdisciplinary team members and support shared decision-making
• Offer consultation on hospitalized patients within their panel
• Support practice efficiency, quality initiatives, and clinical workflows

Qualifications

• Master’s degree and national certification (ANCC, AANP, or NCCPA)
• Active, unrestricted MA NP or PA license, DEA, and MCSR
• Minimum of three years managing a primary care panel, including preventive, chronic, and longitudinal care for adults aged 65+
• Experience with inbox management, test follow-up, messaging, and referrals

Preferred Skills

• Experience in value-based, population health, or team-based primary care models
• Managing complex adult patients with multiple chronic conditions
• Proficiency with clinical pathways, quality initiatives, and Epic EHR

Pay

$165,000 to $190,000
