About the Role
The Primary Care Provider - Advanced Practice Clinician (APC) independently manages a panel of primary care patients, delivering comprehensive, longitudinal care in a team-based, value-driven environment.
Responsibilities
- Manage a defined patient panel with relationship-based, ongoing care
- Perform assessments, diagnoses, and treatment in ambulatory settings
- Coordinate with healthcare team members and support care continuity
- Handle inbox duties like test results, messaging, medication management, and referrals
- Support practice initiatives, quality improvement, and clinical workflows
- Provide consultation for hospitalized patients within their panel
Requirements
- Master’s degree with certification (NP or PA)
- Active unrestricted license, DEA, MCSR, and BLS
- Minimum of three years' experience managing a full primary care panel, including preventive, longitudinal, and chronic care, especially for adults 65+
Preferred Skills
- Experience in value-based care
- Complex adult management
- Team-based primary care
- Familiarization with clinical pathways and Epic EHR
Pay
Competitive salary ranging from $165,000 to $190,000, plus incentives.
