Primary Care Clinician - Advanced Practice (NP or PA)

About the role

We are seeking an experienced Advanced Practice Clinician (NP or PA) to independently manage a primary care panel in a collaborative, value-based environment.

Responsibilities

• Provide comprehensive, longitudinal care for a defined patient panel
• Perform assessments, diagnoses, and treatments in an outpatient setting
• Manage acute and chronic conditions, care plans, and follow-ups
• Coordinate with interdisciplinary team members and support shared decision-making
• Handle inbox responsibilities such as test results, messaging, and referrals
• Partner with patients and families to promote whole-person health
• Support practice efficiency and quality improvement initiatives

Requirements

• Master’s degree and national certification (for NP or PA)
• Active, unrestricted license and DEA/MCSR registration
• Minimum of three years managing a primary care patient panel independently
• Experience with preventive, longitudinal, and chronic disease care, including adult patients over 65
• Proficiency in inbox management, test follow-up, messaging, and referrals

Preferred Skills

• Experience in value-based or population health models
• Managing complex adult patients with multiple conditions
• Team-based primary care experience with interdisciplinary support
• Familiarity with clinical pathways, quality initiatives, and Epic EHR

Pay

$165,000 to $190,000
