About the role
We are seeking an experienced Family Nurse Practitioner to join a patient-centered outpatient primary care team in a respected clinic. This role offers independent practice with your own patient panel within a team-based PACT model, supported by RNs, LPNs, MAs, mental health, and pharmacy teams. The position is ideal for providers seeking a collaborative, autonomous role in a community with outdoor recreational opportunities.
Responsibilities
- Provide continuity of care in an outpatient primary care setting
- Manage chronic diseases such as diabetes, hypertension, and hyperlipidemia
- Work with a veteran population, including geriatric and multi-morbidity cases
- Utilize CPRS EMR for patient management
Requirements
- Family Nurse Practitioner certification
- Minimum of 2 years of outpatient experience
- Comfort practicing independently
