About the role
We are seeking an experienced Family Nurse Practitioner (FNP) for an outpatient primary care role in a collaborative, patient-centered environment.
Responsibilities
- Manage chronic diseases such as diabetes, hypertension, and hyperlipidemia
- Provide geriatric care
- Deliver veteran-focused, continuity-driven care
- Serve as an independent provider with a personal patient panel within a team-based PACT model supported by RNs, LVNs/LPNs, MAs, and mental health and pharmacy teams
Requirements
- FNP certification
- Minimum 2 years outpatient primary care experience
- Strong chronic disease management skills
- Comfort working independently
Veteran or VA care experience is a plus.

Schedule
Monday–Friday, 8:00 AM – 4:30 PM, with options for a 4-day workweek. No nights, weekends, or holidays.
Work Environment
Supportive team in a community close to outdoor recreation and regional airports.
