Primary Care Case Manager (Bilingual English/Spanish)

Duties

• Provide primary care case management services for project-eligible clients
• Expand HAP's network of volunteer family practice physicians
• Process charity care/pharmaceutical applications
• Aid HAP clients with community-based social services
• Quarterly report on clients served and specific outcomes

Requirements

• One of the following: Bachelor's degree in social work, psychology, public health, or a related field and at least one year of experience providing direct case management services to a comparable client base.
• A minimum of 90 semester units with an educational major in social work, psychology, public health, or a related field and at least three years of direct case management experience.

Skills

• Bilingual skills in English and Spanish
• Advanced computer literacy in Microsoft Office (Word, Excel, etc.)
• Well organized, multitasking ability, and excellent written and verbal skills
• Experience working in a health care setting with diverse populations preferred
