About the role

The Case Manager is responsible for assessing the needs of unaccompanied children in care, developing Individual Service Plans, screening for human trafficking concerns, facilitating the safe and timely release or discharge of children and youth, and documenting the provision of services in the case files.

Responsibilities

  • Oversees all aspects of the UAC case
  • Maintain case file documentation in accordance with ORR requirements
  • Maintain confidentiality when gathering, documenting, consulting or consolidating information necessary for the case management assessment and the plan of care
  • Maintain regular contact with each UC, UC’s family when possible, and established service providers to communicate case planning and progress
  • Provides client services as needed, including but not limited to making medical appointments, finding resources, etc…
  • Gather historical and current data relative to completion of a comprehensive case management assessment including, but not limited to, school records, medical records, mental health treatment, legal status and court involvement, social and family strengths and needs, support systems, and specific cultural issues
  • Presents case to federal stakeholders in weekly Case Staffing Meetings
  • Participate in team meetings, staff in-service training and department meetings
  • Carry a case load of 8 clients, varying on program census
  • Completes Significance Incident Reports (SIRs), per federal policy requirements
  • Conducts internal chart reviews to monitor documentation compliance
  • Completes, timely and accurate timesheets and other personnel/administrative records as required
  • Other duties as assigned

Requirements

  • Bachelor’s degree in the behavioral sciences, human services or social services fields
  • Bi-lingual in Spanish and English, written, read and verbal
  • Able to work shift hours and days, as assigned
  • Pass a criminal history background check though Morrison and a suitability determination investigation through the U.S. Office of Personnel Management
  • Proof of Vaccination (MMR, Varicella, DTap & Influenza annually)
  • Able to escort youth by airplane
  • Current CPR and Standard First Aid required
  • 40 hours of required training per year

  • This position requires ORR approval; exceptions to the minimum qualifications require ORR’s explicit written approval prior to hire and ORR may require supervision plans and additional training
