Education/Experience
- HS degree with six (6) years of direct service experience in the Mental Health field, OR
- Associates degree with four (4) years of direct service experience in the Mental Health field, OR
- Bachelor’s degree with two (2) years of experience working in the mental health field.
Note: This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.
How You Will Make a Difference (Job Overview)
This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families. Sessions are in the community, clinic, home and some telehealth. We provide services to individuals and families throughout Santa Clara County.
