Role Overview

The Primary Care Patient Care LPN supports primary care providers to deliver safe, effective, and patient-centered care.

Responsibilities

• Collaborate with patients, providers, and the care team to improve quality outcomes
• Perform clinical tasks within scope
• Ensure accuracy and attention to detail

Qualifications

• Graduate of an accredited practical nursing program
• Licensed as an LPN in the practice state

Skills and Abilities

• Strong communication, teamwork, organizational, and interpersonal skills
• Customer service orientation
• Flexible, positive attitude
• Self-directed with minimal supervision

Preferred Experience

• At least 6 months clinical healthcare experience, preferably in primary care

Physical Requirements

• Ability to lift 50 pounds
• Stand and walk for extended periods
• Perform tasks requiring reaching, bending, kneeling, twisting, and squatting
