About the Role

The Primary Care Patient Care LPN provides safe, patient-centered care under the guidance of the primary care team, collaborating with providers and staff to achieve quality outcomes.

Qualifications

• Graduation from an accredited practical nursing program
• State licensure as an LPN
• Preferred: 6 months clinical experience in primary care

• Flexible, positive attitude, and attention to detail
• Ability to perform tasks independently with minimal oversight

Physical Requirements

• Ability to lift, push, or pull 50 pounds
• Prolonged standing and walking
• Ability to reach, bend, kneel, twist, and squat
