Primary Care Assistant - LPN

Role Overview: The Primary Care Patient Care LPN works under the primary care operations team to deliver safe, effective, patient-centered care in collaboration with providers and the clinical team. Responsibilities: Partner with patients and team members to improve quality outcomes, perform tasks consistent with organizational standards, and support a systematic approach to care delivery. Qualifications: Graduate of an accredited practical nursing program, licensed as an LPN in the state, with preferred healthcare experience in a primary care setting. Skills & Abilities: Strong teamwork, excellent communication, customer service focus, organizational skills, attention to detail, flexibility, and ability to work independently within scope of practice. Physical Demands: Ability to lift 50 pounds, stand and walk for extended periods, and perform tasks requiring bending, reaching, kneeling, twisting, and squatting.
