PNW PACE Home Care Coordinator Case Manager
Position Summary
The Home Care Coordinator Case Manager (HCC) assesses the in-home care needs for PACE participants and coordinates with internal and external resources to ensure appropriate care is delivered. The HCC provides in-home case management to PACE participants and provides oversight to home care aides to ensure that the participants in-home plan of care is effectuated.
Responsibilities
- Assesses in-home care needs by doing an in-person assessment in the participant’s home on admission and routinely thereafter
- Coordinates appropriate care with internal and external care providers (home care aides)
- Provides oversight to the plan of care by observing care and reviewing records
- Covers with compliance to ensure that caregivers are following the plan of care
- Afford for change of condition and completes new assessments as needed
- Provides case management for in-home care needs for supplies/equipment and supports changes to plan of care when there is a change in participant status, i.e. acute illness or injury
- Cover with the Inter-Disciplinary Team to ensure that the established plan of care is effectuated
Requirements
- Two years of experience in case management or care coordination in healthcare setting or community agency
- Advanced degree in health field preferred i.e. MSW
- Licenses in health field preferred i.e. PTA, COTA, OT, PT, LCSW, LPN, RN

Pay and Benefits
We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $26.56 - $38.22 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
