Care Coordinator Case Manager

About the role

RIDGEVIEW Behavioral Health Services is a not-for-profit community mental health center seeking a Full Time Bachelor’s Level Tennessee Health Links Care Coordinator / Case Manager to support clients in the LaFollette, TN area.

Responsibilities

• Care coordination and case management
• Directly work with adult and youth clients, their families, and community resources for mental and physical health
• Secure necessary services to meet client needs

Requirements

• Bachelor's degree
• If Bachelor's isn't in social sciences or mental health (counseling, psychology, social work, vocational rehabilitation), or includes 15+ credit hours of related courses on transcript, relevant training and supervision will be provided after hire
• Prefer experience working with individuals with severe and persistent mental illness
• Maintain a valid TN driver's license and a safe driving record
• Self-starter with excellent time management skills
