About the role
The Care Coordinator (Case Manager) oversees a caseload of HCBS members receiving in-home support, coordinating member care, scheduling shifts, creating care plans, and orienting new staff to UCPSA's members.
Responsibilities
- Oversee a caseload of HCBS members receiving in-home support
- Schedule shifts and create care plans
- Orient new staff to UCPSA's members (clients)
- Handle standby availability, some weekends, evenings, and a flexible schedule
- Provide excellent customer service and make decisions under pressure
- Prioritize tasks, problem-solve, and ensure effective and efficient delivery of care services
Requirements
- 18 years of age or older
- Ability to obtain a current CPR/First Aid certification
- Ability to obtain fingerprint clearance/DES background check
- Ability to obtain a current T.B. clearance card
- The ability to work in a team environment
- The ability to work a flexible schedule
- The ability to learn and operate UCPSA program software
- The ability to lift and transfer clients
- Own/have regular access to a vehicle for client visits and transportation
- Proof of current registration and insurance of the vehicle must be kept on file with UCPSA
- A degree in a related field, case management experience required, and at least one year of direct care experience, personally or professionally
- Bilingual in English and Spanish required

Schedule
Work 4 days a week, 10 hours a day, and have a 3 day weekend
