Physician (MD/DO) for In-home Visits

About the role

At Signify Health, part of CVS Health®, our mission is to build trusted relationships to make people healthier. We’re hiring physician (MD/DO) contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Responsibilities

• Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barriers to care
• Perform a medication and medical history review, as well as a physical evaluation in the member’s home
• Provide a pre-populated, patient-specific form for each evaluation; no prescribing, ordering labs, or altering treatment regimens
• Make referrals to the individual’s health plan, encourage participation of loved ones, and ensure PCPs are aware of health and overall quality of life

Compensation

• Earn up to $3000 to $5000 each week (average 6–9 visits daily, base rate of $100 per completed assessment)
• Increase per-visit rate up to 135% with the Market Multiplier
• Earn additional compensation via a pay-per-test model for visits where the member qualifies for additional diagnostic and preventive tests
• Malpractice coverage included
• Availability Incentives based on weekly availability

Requirements

• State license required
• Medicaid license or willingness to enroll (with support) strongly preferred
• Independent contractor, 1099 status

Schedule

• Shifts available Monday–Sunday (no nights/weekends required)
• Short-term or long-term basis
• Pre-scheduled visits in members’ homes
