Physician (MD/DO) for In-home Visits

About the role

This position involves conducting in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members as a physician (MD/DO) contractor.

Responsibilities

• Perform comprehensive in-home assessments focusing on health, well-being, and potential barriers to care.
• Review medication and medical history, conduct physical evaluations, and document findings using pre-populated forms.
• Identify health concerns without prescribing medication or altering treatment plans.
• Schedule and complete visits independently, with flexibility to work on a short-term or long-term basis.
• Provide additional diagnostic or preventive testing where applicable.

Qualifications & Skills

• State medical license required; Medicaid license or willingness to enroll preferred.
• Strong communication skills and ability to engage patients one-on-one in their homes.
