Physician (MD/DO) for In-home Visits

About the role

At Signify Health, part of CVS Health®, our mission is to build trusted relationships to make people healthier. We’re hiring physician (MD/DO) contractors to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Responsibilities

• Conduct in-home visits that focus on a person’s overall health and well-being and identify potential barriers to care
• Perform a medication and medical history review, as well as a physical evaluation in a health plan member’s home
• Provide a pre-populated, patient-specific form for each evaluation
• Make referrals to the individual’s health plan, encourage participation of their loved ones, and ensure their primary care physicians are aware of their health and overall quality of life

Requirements

• State License Required
• Medicaid License or willingness to enroll (with our support) strongly preferred

Compensation

• Earn up to $3000 to $5000 each week (Average 6–9 visits daily, base rate of $100 per completed assessment)
• Increase your per-visit rate up to 160% with our Market Multiplier
• Malpractice coverage
• Availability Incentives (based on weekly availability)

Schedule

• Shifts available Monday – Sunday (no nights/weekends required)
• Short-Term or Long-Term basis
• Pre-scheduled visit in-home with health plan members
