Responsibilities

• Provide personalized, direct physical therapy services in patients' homes under the supervision of a licensed PT.
• Assist with exercises, gait training, and mobility.
• Utilize modalities like heat and ultrasound.
• Educate patients and caregivers.
• Monitor progress and document in EMR.
• Collaborate with healthcare team to ensure coordinated care.
• Participate in team meetings and quality improvement efforts.

Requirements

• Graduate of an accredited PTA program.
• Current license/certification as a Physical Therapy Assistant.
• CPR certification.
• Valid driver’s license and reliable transportation.
