Responsibilities
- Provide personalized, direct physical therapy services in patients' homes under the supervision of a licensed PT.
- Assist with exercises, gait training, and mobility.
- Utilize modalities like heat and ultrasound.
- Educate patients and caregivers.
- Monitor progress and document in EMR.
- Collaborate with healthcare team to ensure coordinated care.
- Participate in team meetings and quality improvement efforts.
Requirements
- Graduate of an accredited PTA program.
- Current license/certification as a Physical Therapy Assistant.
- CPR certification.
- Valid driver’s license and reliable transportation.
