About the role
We are seeking a Licensed Physical Therapist to deliver personalized, one-on-one care to older adults in their homes and senior living communities.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Provide evidence-based therapy to improve mobility and independence.
- Document care within our EMR system.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Incorporate telehealth and virtual care to enhance access and flexibility.
Requirements
- Valid PT license.
- Degree from an accredited program.
- Proficiency with EMR and computers.
- Current CPR certification.
Physical demands
- Lifting and repositioning patients.
- Driving to care locations.
