About the Role

Licensed Physical Therapist providing one-on-one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model.

Responsibilities

• Conduct evaluations and develop individualized treatment plans
• Deliver evidence-based therapy to improve mobility and independence
• Document patient progress via EMR
• Collaborate with interdisciplinary teams
• Educate patients and caregivers
• Incorporate telehealth when appropriate
• Deliver personalized therapy sessions
• Support patient safety and aging-in-place goals
• Participate in a collaborative team environment
• Create and manage your own schedule
• Utilize virtual care options

Requirements

• Valid Physical Therapist license
• Degree from an accredited Physical Therapy program
• Computer and EMR proficiency
• CPR certification
• Ability to meet physical demands including lifting, repositioning, and driving
• Physical stamina for extended walking, standing, and therapeutic activities
