About the Role
Provide one-on-one geriatric care in senior living communities and private residences, including virtual care options.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Deliver evidence-based therapy sessions to enhance mobility, independence, and wellness.
- Document patient progress in EMR and collaborate with interdisciplinary teams.
- Educate patients and caregivers, and utilize telehealth when appropriate.
- Support aging in place through therapy, promote safety, and improve quality of life.
- Build strong patient relationships within a flexible schedule that may include in-home, senior living, and virtual care.
- Engage in ongoing professional growth and utilize advanced technology.
Requirements
- Valid state Physical Therapy license or eligibility.
- Degree from an accredited Physical Therapy program.
- Proficiency in EMR and basic computer skills.
- Current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, and providing assistance during transfers.
