About the Role

Licensed Physical Therapist delivering personalized in-home and senior living care to older adults, utilizing a flexible schedule and virtual options.

Responsibilities

• Providing one-on-one therapy in private residences and senior living communities
• Performing evaluations, creating care plans, and delivering evidence-based treatments
• Supporting patient mobility, independence, and wellness
• Documenting care using EMR systems
• Collaborating with multidisciplinary teams and educating patients, caregivers, and staff
• Utilizing telehealth when appropriate to extend care access

Requirements

• Valid PT license and degree from an accredited program
• Proficiency in computer and EMR use
• CPR certification

Physical Requirements

• Ability to perform moderate physical effort
• Assist with patient transfers and reposition patients
• Drive between locations
• Use PPE as needed
