About the role
Provide one-on-one physical therapy sessions in private residences, senior communities, and through virtual care to older adults.
Responsibilities
- Perform evaluations, develop personalized treatment plans, and deliver evidence-based therapy.
- Support patient wellness, mobility, independence, and safety.
- Document care accurately in EMR systems.
- Collaborate with interdisciplinary teams, including PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote aging in place.
- Utilize telehealth and virtual care to enhance access and flexibility.
Requirements
- Valid Physical Therapy license or eligibility in practice state.
- Degree from an accredited Physical Therapy program.
- Proficiency with computers and EMR systems.
- Current CPR certification.
Physical requirements
- Ability to exert medium effort, assist with patient transfers, reposition patients, and perform therapeutic activities.
- Capable of driving to patient locations.
- Able to wear PPE when necessary.
