Seeking a licensed Physical Therapist to provide personalized, in-home and senior living care, focusing on improving mobility and independence for older adults.

Responsibilities

• Conduct evaluations, develop care plans, and deliver evidence-based treatments to support wellness and functional independence.

• Document patient progress accurately within electronic medical records and collaborate with interdisciplinary teams.

Qualifications

• Valid Physical Therapy license and degree from an accredited program.
• Proficiency in computer use and EMR systems, CPR certification.
• Ability to meet physical demands such as lifting, repositioning, and assisting with patient transfers.
