Physical Therapist (Sign-On Bonus)

We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in their homes or senior living communities using our Geriatric House Calls™ model. Responsibilities Include Conduct evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and overall wellness.Perform therapy sessions, educate patients and caregivers, and document care accurately within our EMR system.Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.Utilize telehealth and virtual care options to enhance access and continuity of care. Qualifications & Skills Valid PT license in applicable state(s) and degree from an accredited program.Proficiency in basic computer and EMR use, current CPR certification.Ability to meet physical demands, including lifting, repositioning patients, and driving to patient locations.
