About the role
Licensed Physical Therapist providing personalized care to older adults in private residences and senior living communities.
Responsibilities
- Perform evaluations, develop treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
- Offer one-on-one therapy sessions in various settings, including virtual care when appropriate.
- Document patient progress accurately in EMR systems.
- Collaborate with interdisciplinary teams.
- Educate patients, caregivers, and staff on safety and aging in place.
Qualifications & Skills
- Valid Physical Therapy license and degree from an accredited program.
- Proficiency in computer use and EMR documentation.
- Current CPR certification.
- Ability to meet physical demands such as lifting, assisting with transfers, and performing therapeutic activities.
