Physical Therapist (Sign On Bonus)

Role Overview

This position involves providing personalized, one-on-one physical therapy to older adults in their homes or senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments within a flexible, clinician-driven model.

Key Responsibilities

• Conduct evaluations and develop individualized care plans
• Deliver therapy sessions supporting wellness, mobility, and functional independence
• Document care in EMR systems in compliance with regulations
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists
• Educate patients, caregivers, and staff on safety and aging-in-place strategies
• Utilize telehealth and virtual care to enhance access and continuity of care

Qualifications & Skills

• Valid Physical Therapy license and degree from an accredited program
• Proficiency with computers and EMR systems
• Current CPR certification
• Ability to meet physical demands such as lifting, repositioning, and providing assistance
