About the Role

Responsibilities

• Complete assessments and evaluations of patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
• Develop and implement a treatment plan based on patient evaluations.
• Provide services within the scope of practice as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
• Instruct and inform the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visit patients according to the Plan of Treatment, complete progress notes for each visit, and submit them per LHC policy.
• Complete all patient evaluations and develop the physical therapy plan of care within state-specific guidelines.
• Report outcomes of evaluations, goals, and anticipated projected frequency of care.

Requirements

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Pay

Pay range: $77,490 - $116,235 annual total cash target pay.

• $44.71 - $67.06 per visit point.
• $37.25 - $55.88 hourly rate.

Pay is based on several factors including but not limited to local labor markets, education, work experience, and certifications.
