About the Role

Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults to improve mobility, independence, and quality of life.

Responsibilities

• Conduct evaluations and develop personalized treatment plans.
• Deliver evidence-based therapy to enhance mobility and independence.
• Document care in EMR systems and collaborate with interdisciplinary teams.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth as appropriate to enhance care access and flexibility.
• Provide one-on-one therapy in various settings (in-home, senior living, virtual).
• Perform assessments, develop care plans, and document accurately.
• Support patient wellness through therapy sessions and education.

Requirements

• Valid Physical Therapist license.
• Degree from an accredited Physical Therapy program.
• Proficiency in computer and EMR use.
• CPR certification.
• Physical ability to perform duties, including lifting, repositioning, and driving.
