Physical Therapist (Sign On Bonus)

We are seeking a licensed Physical Therapist to deliver personalized, one-on-one care to older adults in their homes and senior living communities. Key Responsibilities Include Providing evaluations, developing individualized treatment plans, and delivering evidence-based therapy to support mobility, independence, and wellness.Documenting patient progress accurately within electronic medical records.Collaborating with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.Educating patients, caregivers, and staff on safety, function, and aging in place.Incorporating telehealth and virtual care to enhance accessibility and continuity of care. Qualifications And Skills Valid Physical Therapy license and degree from an accredited program.Proficiency with computers and EMR systems.Current CPR certification.Ability to perform physical tasks, including lifting, repositioning patients, and driving to care locations.
