About the Role

Licensed Physical Therapist providing one-on-one care to older adults in private homes and senior living communities, utilizing a flexible schedule and virtual care options.

Responsibilities

• Conduct evaluations and develop personalized treatment plans
• Deliver evidence-based therapy to improve mobility and independence
• Document patient progress in EMR
• Collaborate with interdisciplinary teams
• Educate patients, caregivers, and staff on safety and aging in place
• Support patient wellness through therapy sessions
• Promote safety and incorporate telehealth when appropriate
• Build strong patient relationships and manage your own schedule

Requirements

• Valid state physical therapy license
• Degree from an accredited physical therapy program
• Proficiency in EMR and basic computer skills
• Current CPR certification
• Ability to meet physical demands such as lifting and mobility assistance
