Physical Therapist (Sign-On Bonus)

About the role

Licensed Physical Therapist providing personalized, one-on-one care to older adults in private residences, senior living communities, and virtual settings.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy sessions to support mobility, independence, and wellness.
• Document care accurately within the EMR system, ensuring compliance with regulatory standards.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Utilize telehealth and virtual care tools to increase access and flexibility.

Requirements

• Valid Physical Therapy license or eligibility in practice state.
• Degree from an accredited PT program.
• Proficiency in basic computer skills and EMR systems.
• Current CPR certification.
• Ability to meet physical demands of in-home care, including lifting, repositioning, and driving.
