Physical Therapist (Sign-On Bonus)

Role: Physical Therapist providing personalized in-home and senior living care to older adults, focusing on improving mobility, independence, and quality of life through evidence-based treatments. Responsibilities: Deliver one-on-one physical therapy sessions in private residences and senior communitiesPerform evaluations, develop individualized care plans, and deliver evidence-based treatmentsSupport patient wellness, functional independence, and safety through therapy and educationDocument care effectively within the EMR system, adhering to regulationsCollaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialistsUtilize telehealth/virtual care to extend access and flexibility when appropriate Qualifications & Skills: Valid Physical Therapy license or eligibility in the practice stateDegree from an accredited PT programProficiency in basic computer and EMR systemsCurrent CPR certification Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, stand/walk for extended periods, climb stairs, and drive to patient locations. PPE may be required.
