Physical Therapist (Sign-On Bonus)

Role: Physical Therapist (PT) providing in-home, senior living, and virtual care to older adults. Responsibilities Deliver one-on-one physical therapy in various settings such as private residences and senior living communities.Perform evaluations, develop personalized care plans, and provide evidence-based treatments to improve mobility, independence, and quality of life.Document patient progress accurately within electronic medical records.Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and fitness specialists.Educate patients, caregivers, and staff on safety and aging-in-place strategies.Utilize telehealth and virtual care options to enhance access and flexibility. Qualifications & Skills Valid Physical Therapy license or eligibility to obtain one.Degree from an accredited PT program.Proficiency in computer use and EMR systems.Current CPR certification. Physical Requirements: Ability to perform moderate physical effort, assist with patient transfers, repositioning, and navigate community settings.
