About the role
Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults.
Responsibilities
- Conduct evaluations and develop individualized treatment plans.
- Deliver evidence-based therapy to improve mobility, independence, and quality of life.
- Document patient progress in EMR.
- Collaborate with an interdisciplinary team.
- Educate patients, caregivers, and staff on safety and aging-in-place strategies.
- Incorporate telehealth when appropriate to enhance care access and flexibility.
Qualifications & Skills
- Valid PT license.
- Degree from an accredited program.
- Proficiency in EMR and basic computer skills.
- Current CPR certification.
- Ability to meet physical demands including lifting, repositioning patients, and driving to patient locations.
